This program is not designed for organizations seeking low-cost, app-based wellness tools, minimal engagement models, or transactional benefits. It is built for employers prepared to address workforce health as a strategic operational priority.

Our programs are framed in Per-Employee-Per-Month (PEPM) terms to support budgeting and financial decision-making—positioning wellness as performance protection.
PHASES
Phase One — Organizational Assessment
Organizational assessment is conducted to establish baseline needs, gaps in existing wellness program(s) and risk indicators. The assessment covers:
· Data collection: Surveys, Questionnaires, Focus groups
- Information gathered revolves around Eight Dimensions of Wellness
· Shared results: Findings are shared with leadership
· Recommendations: Actionable suggestions are provided that align with organizational priorities.
This phase functions similarly to a strategic advisory engagement and may stand alone if the employer elects not to proceed.
Phase Two — Program Delivery
Program delivery involves provided psychotherapy service, exercise training service, and additional services based on collected data findings, all provided by the care team. A care team consists of one (1) Licensed Mental Health professional (LMHP), one (1) Certified Personal trainer (CPT) and Wellness Director. The various roles are as follows:
· LMHP: Provides regularly scheduled (weekly or biweekly) psychotherapy
- Role involves continuously collecting data to identify common matters of concern related to 8 dimensions of wellness
- All emerging themes will be stored electronically for reporting
- Session information will be kept private, aligning with Health Insurance Portability and Accountability Act (HIPAA) and professional license code of ethics.
· CPT: Provides regularly scheduled (weekly or biweekly) fitness training (Individual, Paired/Grouped) keeping a record of progression (strength, endurance, cardiovascular, flexibility) and biometrics will be stored electronically
· Wellness Director: Monitors collected data, presents results to leadership quarterly, makes recommendations of additional services based on findings
Fitness Training
We recognize many desire having a personal trainer, but it can be costly. We recommend cost sharing! Split the cost with a friend and get a great workout!
Training is $125 for one hour. Half hour sessions are available.

Employee Wellbeing Program (EWP)
Don't have the time to address stressors in your life during the week? Concerned about the cost to obtain professional consultation to tackle an issue?
HWM is proud to offer an employee wellbeing program (EWP) for professional organizations. This service is offered as an employee benefit; NO COST to the employee.
There are numerous providers that offer employee assistant program (EAP) and wellness services, as well as employers offering exercise incentive programs. However, either utilization is commonly low or the program is minimally effective in helping or improving the health of employees. We are confident we can do better!
Our program is enticing. Our program will address long-term issues, yield a better return on investment (ROI), and create a habit a preventative care.
